Request a New Hampshire Offender Report

New Hampshire RSA 106-B:14 requires the State Police division to maintain public criminal-history record information in an electronic database. The statute allows an individual to obtain their own record and allows a person to obtain public criminal-history record information on another person for a fee.

Steps

Prepare a New Hampshire request

    Define the person

    Use the best identifying information available.

    Use State Police form DSSP 256, Criminal History Record Information Release Authorization Form. Section I requests name (including maiden name), address, date of birth, hair and eye color, sex, driver’s-license number and state, signature and date; select the purpose of the record request.

    Define the purpose

    Personal review, general public history, and employment/licensing can have different requirements.

    For your own record requested in person, the form calls for Section I. For release to a third party or any mail request, complete both Sections I and II, including the recipient and authorization signatures. The current form marks the notary signature as not required; RSA 106-B:14 also prohibits requiring notarization for release of public criminal-history information. Do not treat this as the rule for every separate confidential-record or fingerprint program.

    Use the State Police route

    Follow the current official form/process rather than a private aggregator.

    Download DSSP 256 from State Police. The form lists a $25 fee for each request, payable by check to State of NH - Criminal Records, and asks for a self-addressed envelope to avoid delay. Address the request to Criminal Records Unit, 33 Hazen Drive, Concord, NH 03305. The form does not promise a routine delivery deadline.
